Make
An Appointment?
You may telephone or call into reception to arrange an appointment with the doctor or the practice nurse. You will be offered the first routine appointment that is available. If you wish to see a particular doctor you may have to wait longer to be seen. As this is a group practice, you may see any of the partners.
If you have a genuinely urgent medical problem that you feel needs attention on the same day, explain this to the receptionist and you will be offered an opportunity to speak to the duty doctor or, where
appropriate, to be seen that same day. Please do not abuse the urgent appointment system with non-urgent problems, as these are best managed during normal surgeries, when we can give you more time.

Chaperone
If you would like a chaperone to be present during your consultation examination, please ask at reception.
Recording Telephone Calls
Patients should note that for their protection and better care, and for the protection of staff, telephone calls may be recorded. These recordings will be treated confidentially.
Temporary Residents
If friends or relatives require medical attention whilst residing with you, we can treat them as a temporary resident. If you require medical attention while outside our practice area, you can contact any doctor practising in that area and be treated as a temporary resident. If you are out of the area for more than three months, you will have to register elsewhere permanently.
Overseas Visitors
EU residents who can prove their entitlement to reciprocal health care can be seen by the doctor or nurse without charge.
Overseas visitors will be charged for seeing the doctors or the nurse. A refund should be obtained from the visitor’s health insurance company when they return home.

Obtain A Home Visit?
We visit patients at home when they are medically too ill to come to
the surgery or are housebound. It is rare for a child to need a home
visit. We can see six patients at the surgery in the time it takes to
visit one person at home. Requests for home visits should be made as early as possible and preferably before 11.00am. The receptionist will ask for details of the problem.
Please note, lack of transport is not a valid reason for a home visit.

Obtain A Repeat Prescription?
• Drop your repeat slip into the surgery (or post it or fax it), allow 48 working hours for processing, then collect it from the surgery (or if you send a stamped addressed envelope we will post it back to you - allow time for postage). Then take your script to a pharmacy of your choice.
We now have an online system for repeat prescriptions.

Note - telephone requests will not be accepted.
The prescription can be posted to you if you enclose an SAE. The prescription may be collected after two working days, during surgery opening times.
The local pharmacies offer a collection and delivery service for prescriptions – please arrange this with the pharmacy of your choice and make a note on your request slip.
If you are asked to make a review appointment, please do so before your next prescription is due.

Access My Information?
For reasons of confidentiality, results of tests can only be given to the patient (or the patient’s guardian where the patient is a child), unless we have the prior consent of the patient.
The information we hold on you is only accessed on a “need to know” basis by our staff. If you would like to access your own medical records, please put your request in writing and address it to the practice manager.
There is no charge to see your records, but there may be a charge if you require copies of any of the information.
Members of the public have the right to know how public services are organised and run, how much they cost, and how the decisions are made. For a copy of our publication scheme, please write to the practice manager.

Obtain
A Sickness Certificate?
If you are ill and unable to work for up to seven days, you should complete
and forward to your employer a self-certification form (www.hmrc.gov.uk). If you are off work for more than seven days, you will
need to see a doctor to obtain a DSS sickness certificate.
