PARKING
Please note - There is no parking on the forecourt - this
is reserved for the doctors' use only. Free public car parking is available
within walking distance.
Please be aware that the doctors require access to the forecourt at all times, so please do not block the pavement. We would also be grateful if you did not park in front of our neighbours' forecourts.
HELP US TO HELP YOU
- One person for one appointment. Separate appointments must be made
for each family member.
- Children under 13 years of age should be accompanied by a responsible
adult.
- Requests for repeat presciptions do not necessarily require a doctor's
appointment - please ask the receptionist.
- Missed appointments mean longer waiting times for everyone.
Please notify us if you no longer need your appointnent - somebody else
could be seen. Don't waste our time or yours!
- If your consultation is likely to include an intimate examination and you would like a chaperone, please ask the receptionist when you book.
SUGGESTIONS, COMMENTS OR COMPLAINTS
If you have any comments or suggestions about any aspect of the service
provided by the practice, please contact the practice manager by letter, telephone or email through this form.
We have an established procedure for making a complaint. Copies of
the procedure are available upon request from the reception staff
and are on view in the waiting room.
CONFIDENTIALITY
Everyone working for the NHS has a legal duty to keep information about you confidential and, in line with Department of Health Guidelines, The Caldicott Report and the Data Protection Act we wish to advise you of how we handle information we hold about our patients.
- We ask you for information so that you can receive proper care and treatment.
- We keep this information, together with details of your care, and because it may be needed if we see you again.
- We may use some of this information for other reasons. For example to help us protect the health of the public generally, and to see that the NHS runs efficiently; to plan for the future; to train staff, and
account for actions taken.
- Sometimes the law requires us to pass on information: for example to notify a birth.
The NHS Central Register for England & Wales contains basic personal information
of all patients who are registered with a General Practitioner. The Register
does not contain clinical information.
You have a right of access to your health records. Everyone working for the NHS has a legal duty to keep information about you confidential.
You may be receiving care from other people as well as the NHS. So that we can all work together for your benefit, we may need to share some information about you.
We only ever use or pass on information about you if people have a genuine need
for it in you and everyone’s interest. Whenever we can we shall remove details
which identify you. The sharing of some types of very sensitive personal information
is strictly controlled by law.
Anyone who receives information from us is also under a legal duty to keep it confidential.
FREEDOM OF INFORMATION - PUBLICATION SCHEME
Part I: Introduction
This Publication Scheme is a complete guide to the information routinely made
available to the public by Dr Bernard Klemenz, Dr Nathan Moss and Dr Angelo Fortuné -
General Practitioners. It is a description of the information about our General
Practitioners and Practice which we make publicly available. It will be reviewed
at regular intervals and we will monitor its effectiveness.
How much does it cost?
The publications are all free unless otherwise indicated within each Class (with £).
Where information is provided at a cost the charges will be calculated as set
out in Class 7.

Your right to information
In addition to accessing the information identified in the Publication Scheme,
you are entitled to request information about Northern Road Practice General
Practice under the NHS Openness Code 1995.
The Freedom of Information Act 2000 recognises that members of the public have
the right to know how public services are organised and run, how much they cost
and how the decisions are made. The General Practice obliged to respond to requests
about information that it holds and is recorded in any format and will create
a right of access to that information. These rights are subject to some exemptions
which have to be taken into consideration by the General Practice before deciding
what information it can release.
Feedback
If you have any comments about the operation of the Publication Scheme, or how
we have dealt with your request for information from the Scheme, please write
to the Practice Manager (see above).
Part II: Classes of information
All information at the Northern Road Surgery is held, retained and destroyed
in accordance with NHS guidelines. Our commitment to publish information excludes
any information which can be legitimately withheld under the exemptions set out
in the NHS Openness Code or Freedom of Information Act 2000. Where individual
Classes are subject to exemptions, the main reasons are, for example, the protection
of commercial interests and personal information under the Data Protection Act
1998. This applies to all Classes within the Publication Scheme.
The information on this Scheme is grouped into the following broad categories:
1. Who we are
Details of the practice, organisational structures, key personnel and how we
fit into the NHS. This practice has a local Personal Medical Services contract.
The contract is with: Gainsborough Offices, Portsmouth City Teaching Primary
Care Trust, St James' Hospital, Locksway Road, Milton, Portsmouth, PO4 8LD, Telephone
023 9268 4543.
The practice aims to follow National Institute for Clinical Excellence (NICE)
and National Service Framework guidelines. Copies of these can also be found
on the NICE website (www.nice.orq.uk) or
the Department of Health website (www.dh.gov.uk).
The NHS makes up a very large part of the public sector. It is possible to find
out information about our practice, the Portsmouth City Teaching Primary Care
Trust and other NHS services in your area by visiting www.nhs.uk. A full list
of local General Practices can be found there too. The Northern Road Practice
provides general medical services for the geographical area of North Portsmouth,
Widley, Purbrook, Crookhorn and Portchester.
There is a national NHS Plan that explains how the NHS is changing and sets out
how all the different parts function and work together. A copy is available on
the Department of Health website (www.dh.gov.uk/nhsplan).
Some information will be withheld, including personal, confidential information
about individuals which is protected by the Data Protection Act 1998.

3. Funding details and charging policies
Northern Road Surgery receives money from Portsmouth City Teaching Primary Care Trust according to its contract (as per Class 1 above) in exchange for services provided for patients.
The total income received from the NHS before expenses in 2006/2007 was £575,864.
The sum spent on drugs prescribed by Northern Road Surgery in 2006/2007 was £20,376.

There may be circumstances where material cannot be released because it is confidential or commercial information or the appropriate officer designated for these purposes under the Act has taken the view that it may be prejudicial to the conduct of the Practice's affairs.
4. Regular publications and information for the public
Guidance and information leaflets relating to the clinical services and health services that we provide for patients and the public, and our range of regular publications are freely available at the surgery.
5. Complaints
If you wish to make a complaint about any aspect of our service, you should first contact the Practice Manager.
6. Our policies and procedures
These include (but are not restricted to) data protection, prescribing and prescription, zero tolerance and health and safety. Details are available on request free of charge.

7. This publication scheme
In this Class we will publish any changes we make to this publication scheme, the criteria upon which our information management policies are based and a referral point for all enquires regarding information management generally in the practice. We will also publish any proposed changes or additions to publications already available.
Cost of information
For the most part, we will charge you only for hard copies or copying onto electronic media (eg CD-ROM). Some information is available free, but for others there may be a charge. The charges will vary according to how information is made available. Charges are as follows:
a) Via the General Practice website - free of charge, although any charges levied by the Internet Service Provider and personal printing costs would have to be met by the individual.
For those without internet access, a single print-out can be obtained by post from the Practice Manager.
However, requests for multiple printouts, or for archived copies of documents which are no longer accessible or available on the web, may attract a charge for the retrieval, photocopy, postage etc. We will let you know the cost and charges that will have to be paid in advance. We will not provide printouts of other organisations' websites.
b) Leaflets and brochures - free of charge for leaflets or booklets on, for example, services we offer to the public. A list is available in Class 4.
c) "Glossy" or other bound paper copies, or in some cases a CD-ROM, video or
other mediums are chargeable. Contact the from Practice Manager.
d) Email will be free of charge unless it says otherwise. The charges will be reviewed regularly.
